When the worst happens and a catastrophic accident occurs in the workplace, the injuries are life changing and often include permanent disabilities. An excellent Case Manager can make all the difference to ensure the most positive outcome possible. When a 55-year-old female was involved in a car accident at work and sustained a traumatic right leg amputation, her Sheakley Case Manager, Darla M was there to help.
Darla started her case management plan right away. While the injured worker (IW) was still in the ICU, a field case manager was sent to visit the IW and family and see what needs could be identified. Since that initial contact, the topic of return to work and self- sufficiency had been discussed openly with the IW and her family. By utilizing her critical thinking skill, proactive identification of the IW needs, effective collaboration with treating providers, facilitating timely processing of treatment requests, and supporting the IW’s transition back into society to be self-sufficient and live a productive life in spite of difficult circumstances, Darla’s Case Management skills have helped the IW be motivated!
Only 16 weeks from the initial injury the IW has undergone 5 surgeries, completed her rehabilitation, met all of her goals, is receiving skilled nursing services and is in the early stages of fitting for a right leg prosthetic. Darla participated in a multidisciplinary care conference with the IW, her family and her treating providers and is now making plans to transition the IW from rehab to assisted living. Optimal outcomes are being met as a result of active collaboration with the facility staff through each transition of care level. The IW and Darla are already anticipating participation in Vocational Rehab to facilitate return to work!
